Wisdom grows through the friction of real friendship.
What the labels mean

This proverb describes friendship as something with an edge to it, not merely comfort or companionship. Iron does not sharpen iron by resting against it gently; it requires friction, contact, and pressure applied in the right way.
The word countenance suggests this sharpening shows on the face itself, in alertness, character and expression. Real friendship changes a person visibly over time, for better or worse, depending on who does the sharpening.

Deeper insight
What the passage says in its own setting
The proverb sits among a cluster of sayings in Proverbs 27 about the value of honest relationship over flattering isolation, including verse 6, faithful are the wounds of a friend, but the kisses of an enemy are profuse. The chapter as a whole builds a case that comfortable dishonesty is more dangerous than uncomfortable honesty.
The image assumes two pieces of iron, not iron against something softer. Wisdom literature here pictures mutual challenge between equals, not a one-directional mentoring relationship only. Both parties in a good friendship are shaped by the contact, which is a corrective to relationships where only one person is ever expected to grow or change.
